Simple order form with delivery details
Not every seller needs an online shop. This order form asks for the customer's name and contact details, what they want to order with the quantity on each line, the delivery address, the delivery method and any notes for the delivery.
Who it is for
- Makers, bakers and small producers selling directly
- Local businesses taking orders by message or phone and wanting them in writing
- Clubs and associations that sell items to members
When to use it
Use it when your catalogue is small or changes often. Customers describe their order in their own words, one item per line, and choose between standard delivery, express or pick-up. You read the order, confirm the price and arrange payment in your usual way.

The PDF shows the customer's details, the list of items, the delivery address and method and the notes. It works as an order confirmation the customer keeps, and as a picking note for you.

Questions people ask
- Does it calculate prices or take payment?
- No. This template collects orders as text. If you need priced lines with totals, the quote template with totals is closer to what you want, and payments are not part of Fillfold yet.
- Can I change the delivery options?
- Yes. Edit the choices, add a delivery fee note or add questions such as a preferred delivery day.
- Will the customer get a copy?
- Yes. They can download a PDF of their order at the end, which also confirms that it was received.
