Commission request form for artists and makers
A custom piece starts with a conversation, and this form makes it a productive one. The client gives their contact details, describes the piece they want, the preferred size or format, when they need it, their budget, a reference image, and whether it needs shipping, with the address if so.
Who it is for
- Illustrators, painters, sculptors and printmakers
- Jewellers, woodworkers, potters and other makers
- Anyone who takes custom orders and wants a clearer starting point
When to use it
Put the link on your shop, your portfolio or your social profile. The deadline and budget tell you quickly whether a commission is realistic, and the reference image saves you several messages of explanation.
