Event feedback form with rating and what to improve

The best time to ask about an event is right after it. This conversational form asks for a star rating, how likely people are to recommend it, what they liked (speakers, venue, food, networking or organisation), what could be better and whether they would come again.

Who it is for

When to use it

Send the link by email or show it as a QR code at the end of the event. A short form gets many more replies than a long survey. The yes or no on coming again is a simple, honest signal for next year's planning.

What the form asks

  • How likely are you to recommend it? · requiredScale
  • What did you like?Checkboxes

The PDF your respondents receive

Each response is saved as a short PDF, and the whole set can be exported as a spreadsheet to count what people liked best. Positive comments are good material for next year's promotion, once you have permission to quote them.

Questions people ask

Is feedback anonymous?
Yes, in this template, since it asks for no name or email. Add them if you want to follow up.
Can I add questions about specific sessions?
Yes. Add a question for each session or speaker if you want detailed ratings.
How is it different from the session feedback form?
This one covers a whole event. The session feedback form is for a single session or workshop.
