Session feedback form for coaches, trainers and workshops

Feedback right after a session is honest and specific. This conversational form asks for a star rating, how useful the session was on a scale, what worked best and what to change, and whether the person would book another session.

Who it is for

When to use it

Send the link at the end of the session, or in the follow-up message the same day. It takes about a minute. The final yes or no on booking another session is a quick and telling measure of how it went.

Questions people ask

Is it anonymous?
Yes, in this template: it asks for no name or email. Add those questions if you want to follow up with individuals.
Can I use the answers as testimonials?
Only with the person's permission. The testimonial template asks for it explicitly, which is better for that purpose.
How is it different from a course feedback form?
This one is built for a single session or workshop. The course feedback form covers a longer programme.
