Car rental request form with dates, vehicle and licence

A rental starts with a few essential facts. This form asks for the customer's contact details, the pick-up and return dates, the vehicle type (small, family, van or premium), whether there is an additional driver, a photo of the driving licence and a signature.

Who it is for

When to use it

Put the link on your website or send it in reply to an enquiry. You see whether the dates and the vehicle fit and check the licence before the customer arrives, which shortens the handover.

Questions people ask

Does it check availability or take a deposit?
No. It collects requests. You confirm availability and handle payment and deposit yourself.
Is checking a licence photo enough?
It is a first step. Many rental firms also verify the original licence at pick-up, and some countries require it.
Can I collect a second driver's licence too?
Add another upload question for the additional driver.
