Coaching intake form: goals, history and agreement

The first session is better when you already know the client's story. This intake form asks for their contact details, what they want to achieve, why now, how committed they are, which areas they want to work on and what they have tried before. It ends with a signature agreeing to your coaching agreement.

Who it is for

When to use it

Send it as soon as a client books, so the answers are in your hands before you meet. Each question is shown on its own card, which invites thoughtful answers, and the commitment scale gives you an honest signal about readiness.
