Membership application form for clubs and associations
Joining a club should be simple for the member and clear for the committee. This form asks for the applicant's contact details, date of birth and address, the membership type (standard, student, family or supporter), why they want to join, their acceptance of the club rules and a signature.
Who it is for
- Sports clubs, hobby groups and cultural associations
- Neighbourhood groups and societies
- Small communities and professional networks
When to use it
Put the link on your website or hand it out at events. Replace the membership types with yours and mention the fee in the introduction. The committee gets every application in the same shape, with the rules accepted and signed.

Questions people ask
- Does it collect membership fees?
- No. Payment is not part of Fillfold today. Ask for the fee separately, by bank transfer for example.
- Can I approve or reject applications?
- You decide outside Fillfold and tell the applicant. The form only collects the applications.
- What about minors?
- The template asks for a date of birth. If minors can join, add a parent or guardian question and check your local rules.
