Freelancer details form: collect what you need to pay and contract
Before you can pay a freelancer, you need their details in one place. This form asks for their name or company, email, address, VAT or tax number, the bank account (IBAN) for payment, an optional ID or company registration document and a signature.
Who it is for
- Small businesses and agencies that work with contractors
- Accounts teams who chase missing details before payment
- Organisers who pay speakers, designers or writers
When to use it
Send it once you have agreed to work together and before the first invoice. Having the details in a consistent, signed form avoids errors in payments and gaps in your records. Bank details and tax numbers are sensitive, so collect them only through a channel you trust.

Questions people ask
- Is it safe to collect an IBAN this way?
- Responses are encrypted on the server, and only the owner of the form can open them. Even so, limit who has access and delete details you no longer need.
- Does it check the VAT number or the IBAN?
- No. It records what the person types. Verify numbers through an official service if it matters.
- Is it a contract?
- No. It collects details and a signature. The terms of the engagement belong in a separate agreement.
