Volunteer hours log with signature for a certificate

Volunteering deserves a record. This compact form asks the volunteer for their name, the date, the number of hours, what they did and a signature. Each entry is a dated, signed line in the story of someone's contribution.

Who it is for

When to use it

Ask volunteers to complete it after each shift, or once a month. The records are useful for funders' reports, for references and for students who need proof of community service.

Questions people ask

Does it add up the total hours?
Not in the form. Download the responses as a spreadsheet and add up each person's hours.
Does it produce a certificate?
No. The signed entries are your evidence. You write the certificate or confirmation yourself.
Who confirms the hours?
The volunteer signs their own entry. If you need a supervisor's confirmation, add a second signature question.
