NPS survey: the recommendation question with a follow-up
The Net Promoter Score rests on one question: how likely are you to recommend us to a friend or colleague? This form asks it on a scale, and follows with a single open question about the main reason for the score. Two questions, nothing more.
Who it is for
- Small businesses starting to measure customer loyalty
- Product teams that want a simple recurring pulse
- Service providers who need a quick check after a project
When to use it
Use it at regular moments, for example a few weeks after onboarding or at the end of each quarter, so you can compare answers over time. Keeping it to two questions respects people's time and raises the share who answer.

Every question can be changed, removed or reordered before you publish.
The PDF your respondents receive
Each answer is saved as a short PDF with the score and the reason. The scores themselves are easy to count in the spreadsheet export, where you can work out your own promoters, passives and detractors.

Questions people ask
- Does it calculate the NPS for me?
- No. It collects the answers. You can download them as a spreadsheet and calculate your score there, or read the comments one by one.
- Can I add more questions?
- Yes, though the strength of this template is its brevity. If you need more, the customer feedback template is a fuller alternative.
- Do respondents have to give a name?
- No. The template does not ask for one. Add a name or email question if you want to follow up with individuals.
