Travel request form: dates, purpose, cost and approval
A trip is easier to approve when the request is complete. This form asks for the traveller's contact details, the destination, the departure and return dates, the purpose, the estimated cost, what is needed (flight, train, hotel or car) and a signature.
Who it is for
- Companies, associations and schools that approve travel
- Managers and finance teams who want requests in one format
- Teams that plan trips for staff, members or volunteers
When to use it
Ask people to submit it before booking anything. The estimated cost and the purpose let a manager decide quickly, and the list of needs tells whoever books what to organise.

The PDF your respondents receive
The PDF is a signed travel request that the approver can read in a minute and file. It does not approve itself: approval is a decision that you make outside Fillfold.

Questions people ask
- Does it book flights or hotels?
- No. It collects the request. Someone books the trip once it is approved.
- Can I add a budget code?
- Yes. Add a question for a project or cost centre, as in the expense claim template.
- Does it connect to expense claims?
- Not automatically. After the trip, the traveller can claim costs with the expense claim templates.
