Consent, complaint and admin form templates with signature
Some paperwork exists to prove that something happened: a person agreed, a request was made, an incident was reported. These templates collect the facts, the date and a drawn signature, and give you a PDF you can keep.

Choosing the right one
Use the general consent or terms acceptance when someone agrees to your wording, the data access request to handle privacy rights, the complaint and incident forms to record what went wrong, and the authorisation letter when one person acts for another.
- Write the statement people agree to clearly, and keep one subject per form
- Rules on signatures, privacy and reporting differ by country, so check what applies to you
- Delete records you no longer need, especially identity documents
Questions people ask
- Are the signed PDFs legally binding?
- They record a clear agreement with a drawn signature and a date. Whether that is enough depends on your country and what the document is for, so take advice when it matters.
- Is the confidential report form a certified whistleblowing channel?
- No. Some organisations must offer specific channels by law, so check what applies to you.
